What to Expect During Behavioral Health Intake 

During your intake appointment, you’ll meet with a behavioral health professional who will take the time to listen to your story and understand what brings you to treatment. To find out what treatment strategy is best for you, the behavioral health coordinator will ask you a variety of questions. The appointment will cover several areas, including your medical history, symptoms and personal therapy goals. We’ll work to gather a full picture of your emotional, physical and psychological well-being to ensure we recommend the best treatment path for you. 

Some intake questions may include: 

  • Your contact information 
  • Your name, birth date, address and occupation 
  • Your insurance details (if applicable) 
  • The reason you are seeking counseling 
  • Your goals for your behavioral health treatment 
  • Life transitions that may be contributing to additional stress 
  • Any previous behavioral health treatment you’ve received 
  • Your personal and family medical history, including any current medications 
  • Current symptoms, such as anxiety, panic attacks, depression or suicidal ideation 
  • Your personal and family history of substance use, including prescription medications
