Patient Navigator - Adult Partial Care - Full Time - Day - Bridgeton

MAJOR FUNCTION:

The Patient Navigator will work with individuals to support and strengthen their capacity to achieve and maintain their mental health and engage in healthy practices.
QUALIFICATIONS:

Education & Experience:

• Bachelor’s degree in health, psychology, counseling, social work, education or other behavioral health profession required.
• Master’s degree preferred.
• 5 years working with behavioral health required.
• Experience working in an integrated care model preferred.

Certification/Licensure:

• Valid NJ Driver’s license, proof of insurance and have a reliable means of transportation as described in Fleet Policy # A 291 required.
• Safe driving record required.

Knowledge & Skills:

• Bi-lingual in Spanish preferred.
