MAJOR FUNCTION:

The Case Manager facilitates processes daily to expedite patient's care analyzing variations from predicted utilization. Facilitates appropriate use of hospital resources by reviewing all patients admitted to the hospital and evaluating appropriateness of admission and continued stay using approved criteria. Facilitates appropriate reimbursement and pre-certification through ongoing communication to insurance companies and documentation in the patient chart. May assist in facilitating through-put of the emergency rooms and the referral process by being in communication with psychiatrist, LIP, ER, and professionals for acceptance to the CAMHU unit. The case manager will ensure patients are accepted and authorization for services is obtained and will continue with ongoing utilization reviews. Assist with the connection to the next level of care and authorization for those services. The Case Manager will maintain all documented contacts and authorization in the patient chart. The Case Manager collaborates with social work on disposition, aftercare referral, and mental health court needs. The Case Manager will take part in the treatment team, documentation in the plan of care, and running therapeutic nursing group on readiness for discharge planning.

QUALIFICATIONS:

Education & Experience:

RN (BSN within 2 years)/BSW or BA in related healthcare field required.

One-year previous Utilization Review, Discharge Planning, Case Management experience or charge nurse experience in a behavioral health/substance abuse setting.

Experience with physician and clinical staff practice patterns and operational issues.

Certification/ Licensure:

RN required.

Case Management, Addictions, or Mental Health Certification Preferred.
