MAJOR FUNCTION:Provides a variety of social service needs to the medical patient and caregivers as they transition to the appropriate level of care. Responsibilities include completing psychosocial assessments for specific patient populations including dialysis, mother baby and oncology, collaborating with the interdisciplinary team in discharge planning for high-risk patients. Participates in interdisciplinary rounds; educates patients and provides them as well as their caregivers with appropriate community resources and agencies to help ensure safety upon discharge. The master's Prepared Social Worker will function within the guidelines of the National Association of Social Work and ensure responsibility for continued professional and personal growth.

QUALIFICATIONS:

Education & Experience:

  • Graduate from an accredited School of Social Work with an MSW (Master’s in Social Work) degree.
  • State licensure required by the NJ State board of social work examiners.
  • Licensed social worker (LSW) or licensed clinical social worker (LCSW) depending upon years of social work experience and clinical courses.
  • Minimum of one year of post-master’s in social work in the healthcare setting.

Certification/Licensure:

  • Current licensure as a Social Worker in New Jersey, as an LSW required.

Knowledge & Skills:

  • Must have excellent writing skills and effective communication skills and functions well as part of the team.
