Position Summary
We are seeking a highly skilled and compassionate Family Nurse Practitioner (FNP) to join our Primary Care team. The NP will provide comprehensive healthcare services to a diverse patient population, focusing on preventive care, diagnosis, and management of acute and chronic conditions. This role is ideal for a practitioner dedicated to patient-centered care, with a strong ability to work collaboratively with a multidisciplinary healthcare team.
Position Responsibilities
Includes but is not limited to:
• Conduct thorough patient assessments, including history taking, physical examinations, and diagnostic tests
• Develop, implement, and manage patient-centered treatment plans for acute and chronic health conditions.
• Provide preventive care, including health screenings, immunizations, and counseling on lifestyle modifications.
• Prescribe medications, therapies, and other interventions within scope of practice and regulatory guidelines.
• Coordinate with specialists, therapists, and other healthcare professionals to ensure comprehensive care.
• Educate patients and families on health conditions, treatment options, and disease prevention.
• Maintain accurate and up-to-date patient records in accordance with healthcare regulations and organizational policies.
• Participate in quality improvement initiatives to enhance patient outcomes and practice efficiency.
• Serve as a mentor and resource for nursing staff and other healthcare team members.
Position Experience Required
3+ years experience in primary care preferred
Strong knowledge of primary care, chronic disease management, and preventive healthcare
Ability to work both independently and collaboratively within a healthcare team
QUALIFICATIONS:
- Master’s or Doctoral degree in Nursing from an accredited program
- Current Nurse Practitioner license in NJ
- Certification as a Family Nurse Practitioner (FNP)
- Active NJ CDS and DEA license
- BLS certification
